Insurance
We suggest contacting your insurance plan to verify your benefits. If your insurance requires a referral, it will be your responsibility to contact your primary care provider to make sure one has been issued. Should you need further assistance, please contact our billing department at (316) 261-3130 . Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement.
